PROJECT NOTES :  While juggling a career and a family, I realized that a journal to keep track of my children's health history would be a great idea and couldn't find one I liked. I self published this book as a series. One for an individual child, one for families, and one for patients with a special health concern requiring more doctor visit appointment pages with lots of space to write notes.
